Domino theory – Pioneered by Heinrich, one of the fathers of accident analysis studies, this  theory views accident from t he pint of man and machine relationship, frequency and severity relations, reasons for unsafe act, the role of management in accident prevision, cost of accidents, and the impact of safety on the efficiency of functioning (Simillie, 2005).  Heinrich postulated that more than 88% of accidents are caused by unsafe act by the works, 10% by unsafe working conditions, and 2% attributed to natural disasters, which means they are beyond human control.  This theory conceptualizes accident in terms of resultant human injury.
